Obtaining Birth and Death Certificates
Certified copies of most Missouri birth and death records may be obtained through our office. We can access birth records from 1920 to present and death records from 1980 to present.
WHAT YOU'LL NEED:
One primary identification document that includes your name and photo such as: a driver's license, U.S. military ID card, U.S. passport, work or school ID, etc.
At least two alternate forms of identification (Documents must contain your name and the name of the company or organization issuing the document.)
Payment by cash, check or money order
Completed application form and payment
Completed and notarized application form and payment by check or money order (application must be notarized by an acceptable notary public.)
CERTIFICATES MAY BE OBTAINED THREE WAYS:
1. Walk-in - Same day service is available to walk-in customers at our North and South Office. This is the fastest way to obtain a birth/death certificate. You may complete an application and submit the required fee (please see fee schedule below).
2. Mail - You may download the application. Upon completion it may be sent to our office (along with a self-addressed stamped envelope, photo copy of your drivers license, and application fee). Certificates will be processed and returned to sender within two to six weeks. Information may be mailed to:
